About the position
The Discharge Planner - Transition of Care role is responsible for providing comprehensive discharge planning services for all inpatient admissions at Northeast Alabama Regional Medical Center. This position involves developing and implementing discharge plans that facilitate smooth transitions for patients from the hospital to post-acute care settings, ensuring effective communication among healthcare providers, patients, and families throughout the process.
Responsibilities
- Develop and complete a Discharge Plan including various risk and care assessment documents.
- Document all relevant Discharge Planning work in the Electronic Medical Record.
- Make referrals to appropriate Post-Acute Services based on the completed Discharge Plan.
- Complete daily tracking on all assigned patients using Excel or other electronic tracking tools.
- Participate in data analysis and performance improvement activities and meetings.
- Provide effective hand-off to Transition of Care Navigation Services for high-risk populations.
- Act as a liaison between the hospital and post-acute providers for high-risk populations.
- Organize and facilitate weekly post-acute huddle calls.
- Document all correspondence with post-acute providers in the Electronic Medical Record.
Requirements
- Valid Alabama or MSL LPN preferred; BLS certification provided.
- LBSW required; two (2) years' experience in a healthcare setting.
- Experience in the acute care or post-acute care setting is preferred.
