About the position

The Diversion Case Manager at Community Teamwork Inc. is responsible for providing comprehensive case management services to clients and their families, focusing on advocacy, support, and referrals to help clients overcome barriers to self-sufficiency. The role involves working collaboratively with clients to develop individualized action plans aimed at achieving stable housing and economic independence, while also maintaining accurate case records and data collection.

Responsibilities

  • Advocate on behalf of program clients to help them move forward out of homelessness.
  • Work as part of a team with support staff and clients to assess and redirect individuals to alternative options rather than shelter.
  • Complete intake and assessment to identify clients' strengths, needs, barriers, and goals for shelter diversion or rapid rehousing.
  • Develop short-term plans with clients to obtain stable housing, treatment options, and employment strategies.
  • Create and implement individual action plans within 48 hours of assessment.
  • Transition cases requiring long-term work to fellow staff when necessary.
  • Educate the community about shelter services and encourage appropriate referrals and scheduling of intake appointments.
  • Assist clients in applying for benefits and completing applications.
  • Maintain positive relationships with local landlords, property managers, and social service organizations.
  • Collect and maintain statistical data and submit monthly service outcome reports.
  • Provide support, crisis intervention, mediation, and service coordination.
  • Ensure confidentiality of case records and information.

Requirements

  • High School Diploma or Equivalent.
  • Prior related experience preferred in assessment, advocacy, and case management in a Human Service field.
  • Knowledge and experience working with low-income, culturally diverse, and at-risk families and individuals.
  • Demonstrated knowledge of community resources available to low-income individuals and families.
  • Excellent customer service, organizational, oral, and written communication skills.
  • Ability to manage multiple priorities in a fast-paced environment.
  • Proficient computer skills in basic software and data collection systems.
  • Excellent crisis management skills.
  • Ability to maintain accurate records and confidentiality of information.
  • Excellent attention to detail.

Nice-to-haves

  • Bilingual in English/Spanish, English/Khmer, or English/Portuguese preferred.
