About the position

The Driver/Patient Transportation Coordinator is responsible for safely transporting geriatric and physically limited participants to and from their destinations using regular or specially equipped vehicles. This role is crucial in supporting the health and independence of older adults within the community, as part of the Providence PACE/ElderPlace program, which aims to provide comprehensive care for the elderly.

Responsibilities

  • Pick up and deliver geriatric, frail, and/or physically limited ElderPlace participants to their destinations.
  • Ensure the safe operation of vehicles, including large passenger vans or small buses, based on the needs of the participants.
  • Assist participants with boarding and disembarking from vehicles as needed.
  • Maintain accurate records of transportation schedules and participant needs.
  • Collaborate with healthcare teams to ensure participants receive the necessary support during transport.

Requirements

  • Valid National Provider BLS certification from the American Heart Association within 6 months of hire.
  • 5 years of driving experience.

Nice-to-haves

  • Education to meet certification, license, or registration requirements.
  • 2 years of driving experience, including driving a large passenger van or small bus.
  • 2 years of experience transporting people with special needs.
  • 1 year of experience in transporting individuals with special mobility needs.
