About the position

The Ryan White Eligibility/Non-Medical Case Manager plays a crucial role in improving health outcomes for clients living with HIV/AIDS by providing eligibility determination services and assisting clients in accessing public and private programs. This position involves advocacy, problem-solving, and ensuring clients receive a continuum of care and support while adhering to funding guidelines and reporting requirements.

Responsibilities

  • Conduct eligibility screening for potential Ryan White services.
  • Assist clients in completing the Ryan White application for initial certification and semi-annual recertification.
  • Help consumers fill out the Ryan White Liberty Dental Application for enrollment.
  • Prescreen clients for the HIP program to determine qualification for assistance.
  • Provide EIVF to consumers and refer to HIP for follow-up.
  • Maintain knowledge of non-AHN health care programs such as Medicaid and Medicare.
  • Follow up with clients for missing documents needed for eligibility renewals.
  • Complete data entry and application uploads for eligibility applications and updates in CAREWare A/B and RWISE.
  • Manage an assigned caseload of diverse clients living with HIV/AIDS.
  • Schedule future eligibility appointments for consumers before they leave their appointment.
  • Field inquiry calls and explain program requirements for Ryan White eligibility.

Requirements

  • Bilingual in English and Spanish preferred.
  • Valid driver's license and ability to drive the AHN company car when needed.
  • Pass a background check through the State of Nevada.
  • Obtain Community Health Worker Certification within the first year of employment.
  • Obtain an Exchange Enrollment Facilitator License (EEF) within 90 days of employment.
  • High school diploma or GED required; Associate's degree preferred.
  • One year of experience in health care related social work.
  • One year of general office duty experience such as typing and filing.
  • Two years of experience with databases and software programs like Microsoft Word, Excel, PowerPoint, and Visio.

  • Experience in medical billing and patient coordination preferred.
  • Excellent organizational skills.
  • Excellent communication and customer service skills.
