About the position

The Social Worker Case Manager in Care Coordination at Elliot Health Systems is responsible for providing clinical social work interventions to patients across various age groups, including neonates, pediatrics, adolescents, adults, and geriatrics. This role focuses on addressing personal and environmental issues to enhance the emotional, social, and physical well-being of patients while ensuring the efficient use of healthcare services. The position involves collaboration with multidisciplinary teams to evaluate patient care and coordinate services throughout the continuum of care.

Responsibilities

• Collaborates and participates with the primary nurse and the multidisciplinary team in the ongoing clinical social work assessment of patient's status/care management.
• Assists in the development, implementation, and evaluation of integrated clinical pathways.
• Monitors patient's progress to ensure care is appropriate and timely.
• Develops discharge plans in collaboration with the interdisciplinary team.
• Coordinates referrals and conducts utilization reviews.

Requirements

• Master of Science in Social Work required; NH LICSW preferred.
• One (1) to three (3) years of experience in a health care system, preferably in a hospital setting.
• Excellent clinical documentation skills and successful usage of EMR systems preferred.
