About the position

The Embedded Care Coordinator Supervisor at Novum Health plays a crucial role in overseeing a team of care coordinators within a healthcare environment. This position is focused on managing and coordinating healthcare services to enhance patient outcomes and optimize resources. The supervisor will lead a diverse group of healthcare professionals, ensuring effective communication and collaboration while promoting quality improvement initiatives.

Responsibilities

  • Provide exceptional leadership and team management skills to motivate and engage a diverse group of healthcare professionals in the care coordination team.
  • Demonstrate a strong grasp of women and children's healthcare practices and clinical guidelines to guide the care coordinators effectively.
  • Utilize excellent communication and interpersonal skills to foster collaboration among healthcare providers, staff, patients, and stakeholders.
  • Apply strategic thinking and problem-solving abilities to enhance operational efficiency and drive quality improvement initiatives.
  • Exhibit proficiency in healthcare management systems and technology, including electronic health records (EHR) and patient management software.
  • Display knowledge of healthcare regulations, accreditation standards, and compliance requirements, ensuring adherence within the team's activities.
  • Utilize financial acumen to manage budgets, resources, and revenue generation within the clinic.
  • Leverage strong project management and organizational skills to streamline workflows and enhance overall team efficiency.
  • Analyze and interpret data to inform decision-making, steer performance improvement, and optimize patient care.
  • Embrace a commitment to patient-centered care and continuous quality improvement throughout the care coordination process.
  • Foster relationships with community organizations and stakeholders to strengthen collaborative efforts.

Requirements

  • Bachelor's Degree in a human services field (Behavioral science, child development, community mental health, counseling, social work, sociology, psychology, etc.) is required, or an associate's degree in one of those fields with four years of behavioral healthcare experience.
  • Minimum 3 years experience in case management.
  • Minimum of 3 years of operations experience in healthcare coordination or similar role.
  • Minimum of 2 years experience as a supervisor or manager.

Nice-to-haves

  • Knowledge of chronic health conditions, mental health issues, and social determinants of health to oversee holistic and integrated patient care.
  • Familiarity with community resources, support networks, and social services to guide care coordinators in connecting patients with external resources.
