About the position

The Emergency & Addiction Services Case Manager at Call to Freedom in Sioux Falls, SD, is responsible for providing support and services to individuals in crisis, particularly those affected by human trafficking and substance abuse. This role involves community outreach, crisis response, and case management to empower clients and connect them with necessary resources and treatment options.

Responsibilities

  • Serve as the primary contact for clients in crisis, assisting with emergent needs such as domestic violence shelters and homeless shelters.
  • Coordinate timely services for individuals based on Call to Freedom guidelines.
  • Conduct crisis addiction assessments for new and existing clients and assist with state assessment submissions and referrals to treatment options.
  • Provide mobile response services for clients seeking addiction services in rural areas.
  • Link clients with crisis intervention and appropriate emergency service providers in local and rural communities.
  • Assist outreach teams and partners in connecting services within local and statewide communities.
  • Conduct initial screenings and assessments, recognizing clients' needs for safety, autonomy, and confidentiality.
  • Provide case management and coordination of services to outreach clients in a timely manner.
  • Develop and implement individualized case management goals for clients.
  • Collaborate and communicate with community and rural partners to strengthen service delivery.
  • Maintain documentation and records as required.
  • Provide training to community partners and service providers to increase awareness and referral of victims for services.
  • Be available for flexible hours to provide crisis services, including evenings and weekends as needed.

Requirements

  • Bachelor's degree in Counseling, Social Work, or a related field preferred.
  • Experience with victims of violence, trafficking, trauma, and/or substance abuse is helpful.
  • Certified Addictions Counselor (CAC) or Licensed Associate Counselor (LAC) certification required.
  • Excellent verbal and written communication skills.
  • Ability to adapt to flexible scheduling, including evenings and weekends as emergencies require.
  • Valid driver's license and motor vehicle insurance.

  • Knowledge of substance abuse treatment options and resources.
