About the position

The Emergency Community Support Specialist at The Salvation Army is responsible for providing intensive case management to individuals in need, utilizing a strength-based, person-centered approach. The role focuses on promoting recovery, community participation, and stabilization, while preventing the need for higher levels of care. The specialist will work closely with clients to develop individualized service plans, respond to crises, and assist with various community resources and benefits.

Responsibilities

  • Participate in discharge planning from higher levels of care and establish a therapeutic relationship.
  • Provide pre-discharge transition services from psychiatric hospitals, including teaching daily living skills and assisting with housing searches.
  • Complete comprehensive assessments and develop initial Individual Service Plans (ISP) within specified timeframes.
  • Collaborate with consumers, families, and other providers to ensure effective support systems are in place.
  • Respond rapidly to crisis situations and complete participants' Safety and Aftercare Plans promptly.
  • Assist participants in obtaining benefits such as SSI, housing vouchers, and food stamps.
  • Provide advocacy and accompany participants in the community to assess readiness for discharge.
  • Participate in staff meetings and clinical supervision, contributing to performance and quality improvement.
  • Complete ongoing training as required by the program, including Emergency Protective Custody training and Wellness Recovery Action Plan (WRAP).
  • Drive as an essential function of the position.

Requirements

  • High school diploma or equivalent required.
  • At least two years of lived experience, direct care experience in a human service field, or training in a human service field, or a bachelor's degree in a human service field.
  • Ability to be flexible and manage multiple projects or tasks simultaneously.
  • Knowledge of Microsoft Word, Excel, and email software.
  • Administrative skills including data entry and attention to detail.
  • Good written and spoken communication skills and ability to maintain effective working relationships.
  • Demonstrated ability to handle confidential matters sensitively.
