About the position

The Emergency Community Support Specialist plays a crucial role in providing and coordinating rehabilitative and support services for individuals involved or at risk of being involved in the Emergency Behavioral Health system. This full-time position requires the specialist to work 24/7, ensuring that clients receive the necessary support and services to aid in their recovery and integration into the community.

Responsibilities

  • Develop integrated treatment team plans with clients, updating them monthly or as needed.
  • Facilitate the implementation of clients' goals, including community living skills, daily living skills, interpersonal skills, vocational and educational skills, medication management, and more.
  • Discuss discharge goals with clients and develop transition plans.
  • Conduct required assessments for clients.
  • Arrange and provide transportation for clients to various appointments related to their treatment plans.
  • Develop crisis response and safety plans for each client.
  • Assess client safety and respond to crisis and safety concerns as needed.
  • Coordinate treatment activities with community agencies and treatment providers.
  • Advocate for clients and partner with other service providers.
  • Maintain a caseload while meeting monthly program billing requirements.
  • Attend required meetings, trainings, and supervision sessions.
  • Participate in on-call coverage, including holidays and weekends.
  • Document all services and communications per program guidelines.
  • Participate in Performance & Quality Improvement (PQI) activities as assigned.
  • Abide by HIPAA regulations and ensure client confidentiality.

Requirements

  • High school diploma or GED required; Bachelor's degree in Psychology, Sociology, or related Human Service field preferred.
  • Two years of experience in a Human Services field or coursework in a related area may substitute for a Bachelor's degree.
  • Crisis management experience preferred.
  • Case management experience preferred.
  • Excellent interpersonal, communication, and conflict resolution skills.
  • Strong teamwork and adaptability skills.
  • Excellent time management and problem-solving skills.
  • Ability to work flexible hours to meet client needs.
  • Ability to interact with high-risk behavioral health clients and provide de-escalation skills as needed.
  • Integrity, professionalism, and confidentiality in all interactions.

Nice-to-haves

  • Basic understanding of physical health issues and their impact on mental health/functioning.
  • Knowledge of community organizations related to human services.
  • Demonstrated understanding of and ability to work with people of diverse backgrounds.
