About the position

The Engagement Nurse Practitioner at WellBe is responsible for delivering compassionate, home-based care to frail, elderly patients with chronic and acute illnesses. This role is integral to the interdisciplinary team, focusing on patient engagement, initial assessments, and ensuring comprehensive medical records. The position emphasizes the WellBe mission of providing high-quality, concierge-level care to underserved populations, facilitating patient transitions across various care settings.

Responsibilities

  • Model the WellBe values and mission in patient care delivery.
  • Engage new patients into the WellBe program and support their care needs.
  • Perform initial patient screenings and physical examinations.
  • Create appropriate assessment and care plans, ensuring timely documentation.
  • Collaborate with community teams for initial welcome visits and assessments.
  • Order, interpret, and analyze diagnostic tests, determining follow-up needs.
  • Prescribe and manage medications and treatments as appropriate.
  • Update patient records and ensure accuracy at each appointment.
  • Deliver high-quality care while adhering to company metrics and documentation standards.
  • Serve as a key resource for patients, providing ongoing care information and counseling.
  • Participate in interdisciplinary team meetings and maintain required licensure.

Requirements

  • Master's Degree in Nurse Practitioner studies from an accredited program (required).
  • Active, unencumbered Registered Nurse and Nurse Practitioner licenses in the state (required).
  • Current board certification from AANC or AANP (required).
  • BLS certification and active NPI# as an NP (required).
  • CPR certification (required).
  • Active, unencumbered DEA licensure or eligibility to apply upon hire (required).
  • Previous experience in home care, primary care, hospice, palliative care, or geriatrics preferred.
  • Valid driver's license and access to a car for patient visits (required).

Nice-to-haves

  • Experience in diagnosing and treating chronic and urgent medical conditions of older adults.
  • Experience in panel management, primary care, geriatrics, internal medicine, and palliative care.
  • Home care experience preferred.
