About the position

The Enhanced Care Management Leader is responsible for guiding and supporting a team of Lead Care Managers (LCMs) to ensure high-quality care for members. This role involves collaboration with senior leadership to empower the team with necessary tools, training, and resources while maintaining excellence in care management. The position requires a hybrid work model, with a mix of remote work and in-person member visits.

Responsibilities

  • Oversee and coordinate the duties of a team of Lead Care Managers, including assigning caseloads and responsibilities.
  • Audit charting documentation to ensure accurate and prompt member encounter documentation.
  • Audit telephonic encounters to maintain high-quality care delivery.
  • Ensure adherence to established protocols for child/elder abuse, safety concerns, and mental health issues.
  • Attend in-person meetings with Lead Care Managers, including outreach visits and provider appointments.
  • Empower LCMs by providing necessary tools, training, and resources.
  • Foster a supportive and professional work environment.
  • Accommodate and educate new hires on the LCM role and relevant policies.
  • Conduct weekly meetings to communicate goals and updates.
  • Serve as the initial point of contact for complex member issues, directing LCMs to appropriate resources.
  • Monitor employee performance and implement performance improvement plans as needed.
  • Address employee inquiries and process requests promptly.
  • Offer support regarding community services and resources available to employees and members.
  • Comply with departmental and companywide policies and procedures.
  • Maintain a caseload of up to 10 members, including in-person visits as required.
  • Ensure Lead Care Managers meet monthly member encounter targets.
  • Handle employee administration tasks, including terminations and performance reviews.
  • Provide support for training and retraining to LCMs as needed.
  • Continuously train and update knowledge to meet or exceed role expectations.
  • Identify process issues and make recommendations for improvement.

Requirements

  • Bachelor's degree in healthcare, social work, or related field.
  • Minimum 2 years of experience in care management.
  • Current and valid driver's license.
  • Proof of auto insurance.
  • Distraction-free home workspace with a secure internet connection.
