About the position

The EPD Case Manager at The Family Wellness Center is responsible for providing home and community-based waiver case management services to elderly individuals and adults with physical disabilities. This role involves conducting assessments, developing personalized care plans, coordinating services, and ensuring that recipients receive the necessary support to maintain their health and welfare in the community.

Responsibilities

  • Conduct face-to-face visits and complete comprehensive assessments of recipients' medical, social, and functional status within 48 hours of receiving the EPD waiver.
  • Determine and develop a specific written Person-Centered Plan (PCP) for each recipient within 10 days of receiving the referral, collaborating with an interdisciplinary team.
  • Present the PCP to the recipient or their representative for acceptance and submit it to the Department of Health Care Finance (DHCF) for review and approval.
  • Follow up with selected service providers to ensure that all services are in place and meet the identified needs of the recipient.
  • Assist recipients in selecting approved service providers and securing necessary physician orders for service initiation.
  • Conduct quarterly reviews of the recipient's PCP, documenting service utilization, communication with providers, and problem resolution.
  • Assist recipients in accessing Medicaid-financed services and other necessary supports to maintain community living.
  • Coordinate waiver services to ensure timely and cost-effective delivery, providing supportive counseling as needed.
  • Document monthly in-home visits and telephone contacts, ensuring proper care coordination in DC Care Connect.
  • Develop and implement a utilization review plan to ensure non-duplication of services and evaluate service delivery efficiency.

Requirements

  • Master's degree in social work, psychology, counseling, rehabilitation, nursing, gerontology, or sociology.
  • Current licensure as LICSW, LISW, RN, or LGSW is required.
  • At least 1 year of experience working with the elderly or individuals with physical disabilities.
  • Excellent oral, written, and interpersonal skills.
  • Strong organizational and time management skills.

Nice-to-haves

  • 3 years of case management experience (preferred).
  • Knowledge of community resources and Medicaid services.
