About The Position

ChristianaCare is seeking a candidate for a hybrid position that requires frequent on-site work. The role involves leading the implementation and configuration of the Healthy Planet, Care Management, and VBC/VBPM modules of the Epic electronic health record (EHR) system. The candidate must be available to travel for Epic training the week of 3/31/2025 and must become Epic certified within 3 weeks of completing the mandatory training sessions. Applicants must be currently authorized to work in the United States, as ChristianaCare will not sponsor applicants for this position for an employment visa now or in the future.

Requirements

  • Bachelor's degree in Healthcare, Information Technology, or a related field preferred.
  • 2-3 years of experience working with Epic systems or similar EHR platform required.
  • High School diploma with 5 years of related experience may be considered in lieu of bachelor's degree.
  • Epic certification in Healthy Planet, Care Management, and VBC/VBPM preferred.

  • Experience with population health management and value-based care workflows.

Responsibilities

  • Lead the implementation and configuration of the Healthy Planet, Care Management, and VBC/VBPM modules ensuring they meet the specific needs of clinical departments.
  • Collaborate with clinical and operational stakeholders to gather and document requirements.
  • Customize the Healthy Planet, Care Management, and VBC/VBPM modules to align with the clinical and operational requirements.
  • Design and configure the Epic modules to meet the needs of the healthcare organization.
  • Conduct thorough system testing, validation, and troubleshooting to ensure the system meets quality and performance standards before go-live.
  • Work with registries, reporting workbench, decision support tools, and integration with patient and provider outreach tools.
  • Troubleshoot and resolve issues identified during testing and implementation phases.
  • Analyze current population health management, care coordination, and value-based care workflows and identify opportunities for improvement and system enhancements.
  • Develop training materials and participate in training sessions for end-users.
  • Provide ongoing support and troubleshooting for the system post-implementation, addressing user issues and concerns promptly.
  • Maintain comprehensive documentation of system configurations, workflows, and procedures.
  • Facilitate regular meetings and communication with stakeholders to ensure alignment and collaboration.
