About the position

The Evening Case Manager is responsible for the day-to-day operations of the Transitional Housing Program, providing direct care services to residents, including life skills training and transitional support. This role involves managing the safety and cleanliness of the facility while ensuring compliance with house rules and fostering positive relationships among residents and staff.

Responsibilities

• Respect and maintain resident confidentiality while demonstrating a positive attitude toward residents and staff.
• Perform daily operations of the Transitional Housing program, including processing intakes and referrals for admission.
• Ensure a safe environment and adherence to house rules by residents.
• Promote positive peer and resident relations.
• Coordinate and direct daily resident care operations of the facility.
• Communicate with care providers and referral sources to understand residents' problems and assets.
• Participate in quality improvement initiatives and maintain communication with the Housing Manager regarding house needs and incidents.
• Maintain a continuous physical presence within the house during working hours and respond to requests for assistance.
• Identify opportunities for improvement in all aspects of the house.
• Assist with the implementation of policies and programs.
• Function as a team member and effectively communicate with residents and visitors to resolve inquiries.
• Identify and address unsafe practices and safety issues in the house, reporting incidents to the House Manager.
• Participate in mandatory training such as fire safety and infection control.
• Accept responsibility for personal professional development and demonstrate a desire for personal growth.
• Perform other duties as assigned.

Requirements

• Valid Driver's License
• Valid First Aid Certification
• Experience in time management
• High school diploma or GED
