Responsibilities
- Develop relationships with referral sources.
- Clinically coordinate patient care during the transition from inpatient to home care.
- Assist the sales team in increasing admissions.
- Serve as a liaison between the agency and physicians, hospitals, nursing homes, and other medical facilities.
- Conduct pre-discharge evaluation visits to facilities at the request of physicians or facilities.
- Spend time with patients to promote and explain the services they will receive after discharge from the hospital.
- Evaluate patients and families for appropriateness for home care.
Requirements
- Registered Nurse (RN) license in Florida.
- Experience in home health care or a related field.
- Strong communication and interpersonal skills.
- Ability to work collaboratively with medical professionals and patients.
