About the position

ProHealth Physicians is seeking an experienced Pediatric Nurse Practitioner (APC) to join their team in Mansfield, Connecticut. This role is integral to the Optum Care Delivery team, focusing on providing patient-centered, team-based primary care in a supportive environment. The position emphasizes autonomy in practice, collaboration with other clinicians, and a commitment to improving community health through education and empowerment of patients.

Responsibilities

• Provide direct patient care to a dedicated panel of pediatric patients.
• Educate and empower patients to take ownership of their health.
• Collaborate with other healthcare professionals to ensure comprehensive care.
• Participate in an onboarding and mentorship program for professional development.
• Engage in quality improvement initiatives to enhance patient care.

Requirements

• Master's degree in Nursing (APRN or FNP) or completion of a Physician Assistant program.
• Unrestricted Nurse Practitioner or Physician Assistant license in Connecticut (or ability to obtain).
• Certification as a Family Nurse Practitioner through ANCC/AANP or as a Physician Assistant through NCCPA.
• Unrestricted CSR and DEA license (or willingness to obtain prior to start).
