About the position

The Facilitator/Case Manager plays a crucial role in the Health Services Division by assisting with programmatic and administrative responsibilities. This position aims to empower individuals to become healthy, self-sufficient, and contributing members of the community. The role involves direct interaction with clients, assessing their needs, and coordinating services to support their well-being.

Responsibilities

  • Maintain case history records and prepare reports.
  • Interview clients individually, in families, or in groups to assess their situations and determine required services.
  • Serve as a liaison between students, homes, schools, and various services to help children facing challenges.
  • Develop and review service plans in consultation with clients and perform follow-ups.
  • Address legal issues related to child welfare and assist with hearings.
  • Counsel parents on child-rearing problems and determine further actions if needed.
  • Consult with parents and school personnel to identify and solve behavioral issues.
  • Arrange for necessary medical and psychiatric tests for clients.
  • Refer clients to community resources for various services and provide guidance on applications.
  • Counsel individuals and groups on issues like mental health, unemployment, and substance abuse.
  • Provide or arrange support services to prevent serious problems from developing.
  • Collect supplementary information to assist clients effectively.
  • Ensure a safe and healthy work environment.
  • Assist with the project's day-to-day operations, including managing budgets and documentation.
  • Develop and deliver health education and outreach activities.
  • Build and maintain relationships with staff, partners, and youth participants.
  • Facilitate health education sessions in collaboration with various stakeholders.
  • Promote participation and create a welcoming environment for clients.
  • Maintain effective communication with the Project Coordinator.
  • Coordinate care and ensure integration of health education services with other services.
  • Increase community awareness of Health Services through various outreach efforts.
  • Provide progress reports required for program implementation.
  • Monitor and collect data for evaluation and performance measures.
  • Review outreach education materials and curriculum.
  • Administer surveys and pre/post-tests as required.
  • Maintain confidentiality of participant records.
  • Attend required staff meetings and trainings.
  • Travel for agency business using personal transportation.
  • Maintain regular attendance and punctuality.
  • Perform other duties as assigned.

Requirements

  • Associates or Vocational Degree in Social Work, Counseling, Nursing, or related field required.
  • Bachelor's Degree in Social Work, Counseling, Health Education, or related field preferred.
  • Minimum three years of work-related experience in public health or teaching required.
  • Minimum three years of experience in effective group facilitation and creating successful connections with participants required.
  • Ability to work well with a diverse population and develop collaborative relationships with service providers.
  • Ability to maintain confidentiality and promote diversity and inclusion.
  • Understanding of early childhood care and education, early intervention, and child development trends.
  • Ability to analyze and utilize data sets for service coordination.
  • Strong administrative and organizational skills.
  • Ability to maintain a balance among multiple priorities.
  • Strong customer service skills and self-motivated work ethic.
  • Effective presentation, written, and verbal communication skills.
  • Valid California driver's license and reliable transportation.

  • Bilingual English/Spanish or other secondary language preferred.
  • Proficient in Microsoft Office Suite and related business software.
