In addition to direct case management, the Facility Social Worker will cultivate relationships with various external agencies and individuals, including county social workers, school personnel, and mental health professionals, to coordinate services that enhance the youth's treatment program. The social worker will also support families and friends in complying with service plans and seeking necessary therapy programs. Documentation is a critical aspect of this role, including the completion of placement papers, medical forms, and service plans, as well as attending Child and Family Team (CFT) meetings to collaborate with community partners. The social worker will be responsible for conducting initial intake assessments for youth and non-minor dependents, working closely with the Administrator, Clinical Supervisor, or Facility Manager. The role encompasses a variety of ongoing responsibilities that will be assigned by the supervisor, including daily, weekly, monthly, and quarterly duties. This position is classified as exempt and reports directly to the Executive Director.
