About the position

The FACT Case Manager at the Mental Health Resource Center is responsible for providing treatment, rehabilitation, and support services to adults with severe and persistent mental illness. This role involves working within a multi-disciplinary clinical team to promote recovery, empowerment, and self-determination for participants. The position requires a strong commitment to community-based services, with most duties performed in the field rather than in an office setting.

Responsibilities

• Provides treatment, rehabilitation, and support services in accordance with the FACT model.
• Assures the accuracy, completeness, and confidentiality of clinical records.
• Conducts mental health status assessments.
• Coordinates referrals to community providers and support services.
• Provides assistance with activities of daily living, transportation, communication, and social skills.
• Educates participants about symptoms, medications, and mental health issues.
• Serves as a primary case manager for an assigned group of individuals.

Requirements

• Bachelor's degree in Human Services or a related field.
• 1 year of experience working with adults with severe and persistent mental illness.
• Valid Florida Driver's License and automobile insurance coverage.
• Strong communication skills.
