About the position

The Family Care Manager at EveryMind Inc. is a vital role within the Project Wellness team, focusing on providing direct care management services in a school and community-based program. This position aims to integrate resources and services for youth and families, supporting them in achieving their goals. The Family Care Manager will work closely with various stakeholders, including school staff and community agencies, to deliver comprehensive services and ensure the well-being of clients.

Responsibilities

  • Completes comprehensive needs and strength assessments with youth and families resulting in individualized service plans and goals.
  • Consults and collaborates effectively with colleagues, school staff, and other community agencies to provide comprehensive services for clients.
  • Offers information and referral support for community members and school staff.
  • Provides ongoing social services to clients, including securing needed resources and ensuring compliance with court-ordered requirements.
  • Assists with completing applications for programs and services that align with service plan goals.
  • Maintains compliance with School Based Services contract, including meeting productivity and caseload requirements.
  • Conducts psycho-educational groups, educational workshops, and resource presentations as appropriate.
  • Incorporates evidence-based practices to support students and families with various needs.
  • Maintains accurate and timely documentation of client records and evaluates services provided.
  • Manages funds in assigned budget categories to support service plan and client goal achievement.
  • Assists with seasonal events for the school community.
  • Provides translation and interpretation services for clients as time allows.
  • Utilizes regular supervision with the Manager to evaluate and improve care management services.

Requirements

  • Bachelor's degree in a social service field required.
  • 1 year of successful experience in the human services field required.
  • Experience with case management service delivery preferred.
  • Strong personal resilience and mental health to handle high-stress situations.
  • Ability to interact and work collaboratively in a flexible work environment.

Nice-to-haves

  • Experience with trauma-informed care practices.
  • Knowledge of community resources and services available for youth and families.
