About the position

The Family Caregiver Specialist at Vibrant Emotional Health plays a crucial role in providing support and services to older adults diagnosed with serious mental illness. This position is part of an Assertive Community Treatment (ACT) team, focusing on enhancing consumer success in community living and psychiatric stabilization. The specialist will integrate family goals with team tasks and provide psychoeducation to families, ensuring comprehensive care and support for clients.

Responsibilities

  • Provide direct services and facilitate access to community support services (medical, mental health, substance abuse, social/recreational services, housing, entitlements) within an interdisciplinary team.
  • Manage a caseload of up to 15 seniors with serious mental illness, ensuring comprehensive coordination of care, rehabilitation, and wrap-around support to enhance independence, community connection, and goal achievement.
  • Conduct screenings, intakes, and bio-psychosocial assessments; assist in developing Individualized Service Plans (ISPs).
  • Maintain accurate charting and documentation as per requirements.
  • Provide crisis intervention, including emergency hospital admission support.
  • Actively participate in program activities and team meetings, including clinical meetings.
  • Work flexible hours, including evenings/weekends and engage in 24/7 on-call duty on a predetermined schedule.

Requirements

  • MSW, MHC, MFT or related degree; professional license by NYSED preferred.
  • 1-2 years of experience with individuals diagnosed with serious mental illness and co-occurring substance use disorders.
  • Experience with evidence-based practices (motivational interviewing, CBT, Family Therapy, stage-wise treatment).
  • Strong organizational and communication skills, both written and verbal.
  • Proficient in computer literacy, including Microsoft Word, Excel, and PowerPoint.
  • Capable of multitasking efficiently on a regular basis.
  • Exceptional attention to detail and excellent time-management skills.
  • Ability to manage their own task list with minimal supervision.
