About the position

Lincoln Families is partnering with Alameda County's Black Infant Health (BIH) program which aims to empower pregnant and mothering Black women to make healthy choices for themselves and their children as well as build a community that stands up against the injustices towards African-American women and their birthing experiences. The BIH program aims to help reduce stress, build resilience, promote healthy behaviors, and increase social support. This position supports Alameda County BIH's Program Area 1, covering families who reside in West Oakland, Emeryville, North Oakland, and South Berkeley. Reporting to the BIH Clinical Supervisor, the Family Health Advocate position will provide case management by developing and maintaining client Life Plans, assessing progress, and connecting families to community resources. The Family Health Advocate will conduct Life Plan meetings, complete client assessments and Birth Plans, and ensure accurate data entry. Additionally, this role involves managing, facilitating, and organizing group interventions, ensuring timely data entry related to group sessions, and coordinating with the team to align group session goals with client Life Plans.

Responsibilities

  • Support each assigned BIH participant develop a Life Plan based on goal setting that will be followed throughout their enrollment in the program.
  • Provide case management to BIH participants and families, safeguarding the dignity and protecting the rights and individuality of each BIH family.
  • Coordinate services to include intake, assessment, and referrals to internal services and community service providers.
  • Maintain ongoing relationships with community service providers and other agencies/programs to educate and advocate for the needs of the families and communities.
  • Assist and support each BIH participant in the completion of their Birth Plan.
  • Schedule and facilitate weekly prenatal and weekly postpartum Group Sessions ensuring BIH participants attend.
  • Complete home visits with BIH participants and families to ensure fidelity of BIH service delivery.
  • Coordinate and work collaboratively with the BIH team to create alignment with client Life Plans, goals, and group facilitation content and programming.
  • Provide ongoing health education that supports a healthy pregnancy.
  • Assist BIH participants in understanding parent and infant bonding as well as learning and applying effective parenting techniques.
  • Administer BIH participant satisfaction surveys after group sessions and at completion of program for feedback on program improvement and retention.
  • Assist in increasing BIH recruitment, participation, and retention of qualifying participants by helping to develop and implement BIH outreach plan.
  • Maintain the confidentiality and security of client exchange and records, adhere to HIPAA regulations, and ensure accuracy of data entry and reporting.
  • Drive to various locations across Alameda County to deliver services at assigned office(s), in the community or via home visits.

Requirements

  • Lived understanding of African American culture and history.
  • General working knowledge of family systems, community trauma, and community organizing or advocacy.
  • Demonstrated awareness of, sensitivity to and competence in dealing with cultural and socioeconomic diversity of client population.
  • Genuine interest and passion for serving African American families.
  • Excellent interpersonal, communication (verbal and written) and organizational skills.

Nice-to-haves

  • Bachelor's degree in Social Work or human services related field preferred.
  • 3 years of related work experience required.
  • Non-profit, school and/or public systems experience preferred.
  • Prior experience managing program data and generating accurate program reports highly desirable.
  • Current or prior experience/familiarity as an Oakland or Berkeley resident a plus.
