Responsibilities

• Perform routine physical examinations and assessments of patients.
• Formulate medical, educational, and psychosocial plans to treat patients.
• Collaborate with physicians and care team members to develop further evaluation plans for health needs.
• Provide appropriate treatment for health issues within the scope of practice as designated by protocols.
• Refer patients to physicians or specialists when problems are outside the scope of practice.

Requirements

• Unrestricted State of Connecticut Medical License (or eligible).
• Master's degree as a Family Nurse Practitioner (FNP) or Physician Assistant (PA).
• At least two years of post-graduate experience as a Nurse Practitioner (APRN) or Physician Assistant (PA) in a primary care setting.
• Certification as a Family Nurse Practitioner (FNP-C) or Physician Assistant (PA-C).

Nice-to-haves

• Master of Business Administration (MBA).
• Experience with EPIC clinical and informatics technology.
