About the position

Hartford HealthCare is seeking a full-time Family Nurse Practitioner (APRN) or Physician Assistant (PA) for a 100% virtual/remote position. This role involves providing primary care services to patients in Connecticut, utilizing advanced clinical and informatics technology to enhance patient outcomes and provider satisfaction. Responsibilities

• Perform routine physical examinations and formulate medical, educational, and psychosocial plans to treat patients.
• Develop plans for further evaluation of health needs in consultation with physicians and care team members.
• Provide appropriate treatment for problems within the scope of practice and refer issues outside of scope to physicians.

Requirements

• Unrestricted State of Connecticut Medical License (or eligible).
• Training as an APRN (Family Nurse Practitioner) or PA.
• At least two years of post-graduate experience as a Nurse Practitioner (APRN) or Physician Assistant (PA) in a primary care setting.
• Must reside in the state of Connecticut.
