About the position

Shenandoah Community Health (SCH) is seeking a full-time experienced Family Nurse Practitioner to join our outpatient team in Winchester, VA. This position is integral to our community health center, which serves as a Primary Care Medical Home, providing comprehensive access to primary care, behavioral health, women's health, and oral healthcare services in the Shenandoah Valley region of West Virginia and Virginia. Responsibilities

  • Provide comprehensive healthcare services to patients in an outpatient setting.
  • Conduct thorough assessments and evaluations of patients' health status.
  • Develop and implement individualized care plans based on patient needs.
  • Collaborate with a multi-disciplinary team to ensure continuity of care.
  • Educate patients and their families on health management and disease prevention.
  • Maintain accurate and up-to-date patient records and documentation.
  • Participate in quality improvement initiatives and adhere to best practices in healthcare.

Requirements

  • Current and valid Nurse Practitioner license in Virginia.
  • Experience as a Family Nurse Practitioner in a clinical setting.
  • Strong clinical assessment and diagnostic skills.
  • Ability to work collaboratively in a multi-disciplinary team environment.
  • Excellent communication and interpersonal skills.

Nice-to-haves

  • Experience in community health settings.
  • Familiarity with electronic health record (EHR) systems.
  • Bilingual skills, particularly in Spanish, are a plus.
