About the position

The Family Resource Coordinator (FRC) plays a crucial role in assisting eligible children and their families in accessing their rights and services under Washington's early support for infants and toddlers program. This position serves as the primary contact for parents, guiding them through the service coordination process to enhance their child's development and ensure they receive necessary support.

Responsibilities

  • Assist eligible children and their families in accessing rights and services under Washington's early support for infants and toddlers program.
  • Serve as the single point of contact for parents to access services and support.
  • Inform families of their rights and procedural safeguards.
  • Collect referral demographics and developmental information during the intake process.
  • Enter data into the WA ESIT Data Management System and agency electronic health records system.
  • File client paperwork into electronic files and ensure compliance with established protocols.
  • Assist with coordinating evaluations to determine eligibility for the program.
  • Facilitate and participate in the development, review, and evaluation of the Individualized Family Service Plan.
  • Coordinate and monitor the delivery of available services.
  • Inform families of additional resources and supports outside of the program.
  • Collaborate with medical and health providers and community partners as needed.
  • Facilitate the development of a transition plan in collaboration with local school districts.
  • Ensure all early support activities occur within designated timelines.
  • Provide program services effectively and in alignment with the agency's philosophy.
  • Deliver family-centered and relationship-based services.
  • Demonstrate sensitivity to linguistic, cultural, social, economic, individual, and role differences among families served.
  • Maintain an established work schedule and adapt as needed to meet unexpected changes.

Requirements

  • Bilingual in Spanish
  • Master's degree or Bachelor's degree in a relevant field
  • Experience in case management
  • Skills in meeting facilitation
  • Knowledge of child welfare practices
