About the position

The Family Specialist/Home Developer/Case Manager at Pathways Youth & Family Services is responsible for providing ongoing monitoring, support, and case management services to foster and adoptive families. This role involves conducting home visits, ensuring compliance with standards, and facilitating training and support for families. The position requires local travel within the Beaumont area and some in-office work at the Houston location, focusing on the well-being of children and families in the foster care system.

Responsibilities

  • Provide assistance and support to foster families to ensure compliance with DFPS Minimum Standards and Pathways' policies.
  • Assist foster/adopt parents in documenting progress on Service Plan goals.
  • Conduct and document monthly and quarterly visits to assigned foster/adopt homes.
  • Provide training during quarterly visits to foster/adopt homes.
  • Create and ensure compliance with Action/Safety Plans.
  • Respond to internal referrals for placement and prepare families and children for placement.
  • Conduct placement and complete required documentation/training when necessary.
  • Conduct 7-Day Follow-Ups post-placement to ensure child well-being and safety.
  • Schedule and facilitate service plan meetings and develop Service Plans.
  • Monitor foster/adopt homes and services provided to children through monthly contacts.
  • Ensure educational services are provided to children and link assessments to meet educational needs.
  • Attend court hearings and permanency planning meetings, sharing information with legal representatives and treatment teams.
  • Complete monthly documentation on child progress regarding Service Plan goals.
  • Plan, implement, and review behavioral interventions in conjunction with the CPMS, Program Director, and/or Treatment Director.
  • Facilitate initial orientation and ongoing training to families and staff.
  • Participate in recruitment activities for foster-adopt-kinship parents/support providers.
  • Respond to inquiries and maintain accurate applicant lists.
  • Coordinate initial home visits and home studies with contracted Home Study Writers.

Requirements

  • Master's degree in a human services field and one year of supervised child-placing experience, or
  • Master's degree and two years of supervised child-placing experience, or
  • Bachelor's degree in a human services field and two years of supervised child-placing experience, or
  • Bachelor's degree and three years of supervised child-placing experience, or
  • Bachelor's degree with direct supervision from a qualified person.
  • Thorough working knowledge of needs of children placed in substitute care.
  • Effective oral and written communication skills.
  • Competent in using Microsoft Word, Excel, PowerPoint, and other software applications.
  • Ability to work efficiently both individually and as part of a team.

Nice-to-haves

  • Experience in child welfare or social services.
  • Knowledge of local community resources for families.
  • Bilingual skills.
