Positive Leaps is seeking a dedicated and compassionate Community Case Manager to join our team in West Chester, OH. This full-time position is focused on providing essential case management services to families with children aged 3-8 who exhibit negative behaviors or require assistance with emotional regulation. Our Day Treatment Center is designed to resemble a daycare or early learning center, ensuring a nurturing environment where children can thrive. The role involves leading care coordination efforts in collaboration with both internal and external support systems to ensure that treatment is cohesive and effective. The Community Case Manager will work diligently to confirm that necessary supports are available and actively engaged for clients and their families, thereby reducing barriers to treatment and striving for excellent clinical outcomes. As a Community Case Manager, you will be responsible for providing case management services in various community settings, including schools, daycares, and clients' homes. You will assist families with symptom monitoring, case coordination, and resource identification, addressing their social and treatment needs. The position requires facilitating prior authorization and re-authorization processes with third-party payers to ensure financial coverage for clients in treatment. You will serve as a vital point of contact for families, treatment team members, and community support agencies, regularly meeting with supervisors and treatment teams to strategize and address client and family needs. In this role, you will create a safe and supportive environment for clients and their families, interacting with them in a professional and positive manner. Flexibility and adaptability are crucial, as you will be working in rapidly changing environments. You will support prescribed clinical treatments by implementing the Positive Leaps model of care and curriculum, while also maintaining accurate documentation of clinical data in compliance with Ohio laws and agency policies. The position includes responsibilities for crisis intervention services, ensuring ethical conduct, and modeling professional behavior. Responsibilities

  • Provides case management services to families of children in community settings, such as schools and daycares, as well as in the home and virtually as needed.
  • Assists families in symptom monitoring, case coordination, resource identification, and other social and treatment needs.
  • Facilitates prior authorization and re-authorization contact with third-party payers to ensure financial coverage for clients in treatment.
  • Serves as a point of contact for families, treatment team members, collaborative agencies, and community support.
  • Regularly meets with supervisors and/or treatment team to plan and problem solve for client and family needs.
  • Provides a safe space for clients and families.
  • Interacts with children, families, and other staff members in a professional, positive manner.
  • Exhibits flexibility and adaptability in rapidly changing environments.
  • Supports prescribed clinical treatment by teaching and implementing the Positive Leaps model of care and curriculum or any other prescribed intervention.
  • Prepares, updates, and maintains timely and accurate documentation of clinical data in accordance with Ohio laws, third-party payers, and agency policies.
  • Implements and collects data related to treatment goals and objectives.
  • Consistently meets performance measures including competency indicators and participates in quality assurance activities for continuous quality improvement.
  • Assures consistent implementation of Positive Leaps' policies and procedures.
  • Provides crisis intervention services during or after regular business hours, if the need arises.
  • Demonstrates ethical conduct, in keeping with professional standards set by licensee's professional board and Positive Leap's ethical code.
  • Models professional behavior and demeanor, dress, and work space organization.
  • Completes assigned tasks as directed by supervisors and within agency timeframes.

Requirements

  • Bachelor's degree in Social Work with License
  • Must be at least 19 years of age
  • Experience working with children
  • Excellent verbal and written communication skills
  • Ability to work in a very active environment and lift up to 50 lbs
  • Valid Ohio driver's license and auto insurance
  • Willingness to work in various environments to promote treatment (i.e., client home, school, office, and community)
  • Ability to pass a rigorous background check
