About the position

The Family Support Worker II position is a full-performance role focused on providing in-home support services to individuals, helping them maintain independence and well-being. This role involves visiting clients' homes to assess needs, provide assistance with daily living activities, and promote the use of community resources. The Family Support Worker II works as part of a treatment or service team under general supervision, contributing to the development and modification of care plans without supervising other staff.

Responsibilities

  • Provides in-home support services to enable individuals to maintain their independence and well-being.
  • Visits clients' homes to identify needs and provide assistance in daily living activities.
  • Teaches and demonstrates practical living skills such as meal planning, household maintenance, and positive parenting.
  • Assesses clients' personal and living conditions to determine the need for behavioral changes.
  • Develops positive relationships with clients to gain acceptance of the In-Home Care Plan.
  • Encourages clients to utilize community resources and services.
  • Observes and documents client behavior, maintaining records for service teams.
  • Prepares reports on clients' conditions and services provided to assist in care plan development.
  • Participates in team meetings with case managers and health care professionals.
  • Transports clients to medical appointments and community centers.
  • May present training sessions on Parenting or Job Readiness and conduct in-home follow-ups.
  • May assist clients with personal care and provide temporary care in the absence of caregivers.

Requirements

  • High school diploma or GED.
  • Two years of experience providing support services to children or adults.
  • Knowledge of social, physical, and psychological needs of various age groups.
  • Knowledge of behavioral theories and social science related to behavior modification.
  • Ability to communicate effectively with clients and professionals.
  • Ability to teach and demonstrate caregiving and home management skills.

Nice-to-haves

  • Experience in child or adult caregiving.
  • Knowledge of community resources and services.
  • Experience in conflict management and interpersonal relationship skills.
