About the position

The Family Team Conferences Coordinator at Lutheran Social Services of New York is responsible for facilitating Family Team Conferences (FTC) and ensuring the effective delivery of preventive services to children and families. This role emphasizes culturally responsive case management and evidence-based practices, focusing on child safety, well-being, and risk assessment over a period of 6-12 months. The coordinator will track conference activities, communicate recommendations, and provide ongoing education and training related to Family Team Conferencing.

Responsibilities

  • Facilitate Family Team Conferences and Service Termination Conferences.
  • Maintain fidelity to the evidenced-based practices for the program.
  • Hold pre-conferences and debriefings.
  • Track activities related to FTC conference including timeliness and completion rate.
  • Assess the strengths and needs of each child and family member in consultation with the interdisciplinary team and specialty consultants.
  • Ensure all recommendations to FSS case planners and other service providers are communicated and captured in the action plan.
  • Identify, evaluate and address all safety and risk concerns during conferences.
  • Participate in meetings and present FTC data.
  • Complete timely documentation in Connections and related agency databases.
  • Monitor and ensure that all agency facilitated FTCs are documented in connections.
  • Facilitate a program determined minimum of conferences per day.
  • Attend inter/intra agency meetings to share information, develop policies and procedures.
  • Conduct Quality Assurance activities related to Family Team Conferencing.
  • Monitor outcomes related to identifying performance trends, best practices and opportunities for improvement.
  • Collect statistical data on Family Team Conferences and compile and present reports every month.
  • Provide ongoing education and training to clients and staff on Family Team Conferencing.
  • Ensure adherence with the agency, ACS and OCFS policies and procedures.
  • Act as a liaison for translation services.
  • Assess the safety and well-being of clients as appropriate.
  • Assist Case Planners, Social Workers, Supervisors, and Directors as needed.
  • Ensure compliance with local, state, and Federal rules and regulations, including but not limited to HIPAA.
  • Attend Children Services Team Meetings as needed.
  • Serve as liaison for ACS facilitators and LSSNY.
  • Participate in all required LSSNY training.

Requirements

  • Master's degree in Social Work or closely related field preferred.
  • Bachelor's degree in social work, psychology, sociology, or human services required.
  • Two years minimum experience in child welfare or related service area.
  • Proficient in Microsoft Excel, Word, and PowerPoint.
  • Exceptional mediation skills to effectively facilitate group conference situations.
  • Strong clinical skills and strength-based perspective.
  • Strong verbal and writing skills.
  • Excellent organizational skills.
  • Ability to handle multiple tasks at one time.
  • Knowledgeable of core principles and standards for NYC Child Welfare system.
  • Ability to travel in and around New York City.

  • Bilingual (Spanish) a plus.
  • Knowledgeable about substance use and mental health clinical issues.
  • Knowledgeable about working with foster care youth and children and families at risk.

Benefits

  • Full-time position with a salary of $52,500 per year.
