About the position

Mat-Su Health Services is seeking a FEP (First Episode Psychosis) Case Manager/ Outreach Specialist to join our team! We are a Federally Qualified Health Center providing integrated medical, dental, and behavioral health care. Our Mission, dedicated to improving the health of our community, one person at a time-through affordable medical, dental, and behavioral health care, attracts and inspires a caring, competent workforce. Under the direct supervision of the Director of Behavioral Health, the FEP Case Manager/Outreach Specialist provides the initial support for those individuals who may be experiencing their initial episode of psychosis. Case management duties include case finding, case management, assessment and planning, and case coordination between client, family, and community resources. Works to develop and facilitate rehabilitation activities, including skills development and recreational opportunities. Due to the small caseload of the individuals identified for FEP a large majority of the time will be spent performing community outreach to develop referral sources for the program.

Responsibilities

  • Maintain regular contact with outside resources and providers.
  • Advocate on behalf of the consumer, assist qualifying clients/families in identifying, obtaining, and securing needed employment, entitlements, supports, and services.
  • Maintain accurate client service records and distribute completed information and medical records to service providers within the established time frame.
  • Review treatment plans, monitor progress, and provide necessary follow-ups as required by the current Division of Behavioral Health regulations.
  • Provide and participate in interdisciplinary quality assurance, utilization review, treatment planning, treatment assessments, and assessment of function.
  • Monitor and coordinate client services and the service delivery system to ensure the care is appropriate, medically necessary, clinically justifiable, and active treatment.
  • Provide direct services, including individual and group skills development and crisis intervention.
  • Research and develop curriculum and prepare class materials for skills-building classes.
  • Participate in the FEP Team and FEP Team meetings.
  • Perform planned outreach activities with a monthly schedule of places to introduce the program and provide educational materials and other giveaway items.
  • Obtain referrals from a minimum of 3 different sources throughout the year.
  • May provide training as needed.
  • Remain familiar with and adhere to published Agency policies.

Requirements

  • BA/BS degree preferred in Social Work, Psychology, Human Services, or related field with some work experience.
  • 5 years of relevant work experience may be substituted for education.
  • Clear background check (subject to DHSS background check).
  • Current tuberculosis screening test result.
  • Current driver's license; clean driving record history.
  • BLS certification must be obtained within two weeks of hire.
  • CPI (Nonviolent Crisis Intervention) certification must be obtained when offered through the agency.
