About the position

The Care Connections Nurse Practitioners focus on delivering screening and preventive primary care services in non-clinical settings such as homes, communities, and nursing facilities. This role emphasizes providing medical care in environments where patients feel most comfortable, catering to various populations including adults, geriatrics, and pediatrics. The Nurse Practitioner will perform comprehensive medical assessments, order diagnostic tests, formulate treatment plans, and document care appropriately, while also participating in innovative care models and community outreach programs.

Responsibilities

  • Provide general medical care and care coordination to various patient levels including adults, women's health, pediatric, and geriatric.
  • Perform comprehensive evaluations including history and physical exams for gaps in care and preventative assessments.
  • Address both chronic and acute primary care complaints, assessing medical urgency.
  • Establish and document reasonable medical diagnoses and seek specialty consultation as appropriate.
  • Order and perform pertinent diagnostic laboratory and radiology testing judiciously.
  • Create and implement a medical plan of care and schedule patient appointments for in-person visits when appropriate.
  • Provide post-discharge coordination to reduce hospital readmission rates and emergency room utilization.
  • Perform face-to-face visits in various settings including home, skilled nursing facilities, and public locations.
  • Participate in community-based 'Pop Up Clinics' to build relationships and address healthcare gaps.
  • Drive up to 120 miles a day to various locations within the assigned region, with potential for extended mileage on special project days.
  • Collaborate with fellow nurse practitioners to develop best practices for efficient work duties.
  • Prescribe medications and perform procedures as appropriate.
  • Perform timely documentation in electronic medical records.

Requirements

  • Master's degree in family health from an accredited nursing program.
  • An active and unrestricted national certification from a recognized organization (e.g., American Academy of Nurse Practitioners).
  • Current state-issued license to practice as a Family Nurse Practitioner.
  • Current Basic Life Support for Healthcare Professional certification.
  • Current unrestricted driver's license.
  • Advanced computer skills, proficient with Word, Excel, and Electronic Medical Records.

Nice-to-haves

  • 3-5 years of experience as a Registered Nurse and/or Nurse Practitioner, ideally in home health or community health settings.
  • Experience with underserved populations facing socioeconomic barriers to healthcare.
  • Fluency in a language in addition to English.
  • Immunization and point of care testing skills.
