About the position
Advantmed is seeking a highly skilled and compassionate certified Nurse Practitioner to join its provider network. This role involves conducting in-home wellness risk adjustment assessments for Medicare members and other at-risk populations, with a focus on ensuring accurate risk adjustment coding and enhancing value-based care. The position offers flexibility and is structured as a 1099 (PRN) role, allowing for a balance of competing priorities while providing quality care to members.
Responsibilities
- Perform annual in-home wellness visits and risk adjustment health assessments on a population with chronic conditions.
- Conduct comprehensive assessments including physical examinations, vital signs, medication history, and functional status reviews.
- Deliver patient health education on topics such as pain management and medication.
- Build relationships of trust with members through exceptional communication and empathy.
- Assist in closing quality care gaps, such as screenings and labs.
- Collaborate enthusiastically to enhance care delivery.
Requirements
- Must have a valid unencumbered NP License for the state you will be working in.
- Access to reliable transportation for travel within a 55-mile radius of the assigned job posting county.
- Strong ability to work within an EMR system.
- Ability to work independently.
- 3 years of patient care experience preferred (primary care/adult/geriatric, EMR).
Nice-to-haves
- Previous in-home risk assessment experience preferred.
- Previous 1099 (PRN) experience is preferred but not required.
- Bilingual is a plus.
