About the position
The Field Nurse Practitioner role at Advantmed involves conducting in-home wellness risk adjustment assessments for Medicare members and other at-risk populations. The primary objective is to assess the overall health and well-being of beneficiaries to ensure accurate risk adjustment coding, thereby enhancing value-based care. This position is a PRN (1099) role, offering flexibility for professionals balancing various commitments.
Responsibilities
- Perform annual in-home wellness visits and risk adjustment health assessments on a population with chronic conditions.
- Conduct comprehensive assessments including physical examinations, vital signs, medication history reviews, and cognitive assessments.
- Deliver patient health education on topics such as pain management and medication.
- Build relationships of trust with members through exceptional communication and empathy.
- Assist in closing quality care gaps, such as screenings and labs.
- Collaborate enthusiastically to enhance care delivery.
- Commit to a minimum of 30 hours per month.
Requirements
- Must have a valid unencumbered NP License for the state of North Carolina.
- Access to reliable transportation for travel within a 55-mile radius.
- Previous in-home risk assessment experience preferred.
- 3 years of patient care experience preferred, particularly in primary care, adult, or geriatric settings.
- Strong ability to work within an EMR system.
- Ability to work independently.
- Bilingual is a plus.
Nice-to-haves
- Previous 1099 (PRN) experience is preferred but not required.
