About the position

The RN Care Coordinator role at UnitedHealthcare is a vital part of the Integrated Care Model, focusing on advocating for members' healthcare needs and ensuring they receive appropriate services. This position involves working closely with nursing facilities and requires local travel to meet members in various settings, including their homes and community centers. The role emphasizes individualized care strategies and effective communication among stakeholders to facilitate quality care.

Responsibilities

  • Assess, plan, and implement individualized care strategies for members.
  • Identify and initiate referrals for member transitions to community services and assist with social service programs.
  • Manage the care plan throughout the continuum of care as a single point of contact.
  • Communicate with all stakeholders to ensure quality coordinated care and services are provided expeditiously.
  • Advocate for members and families to ensure their needs and choices are represented and supported by the healthcare team.
  • Visit Medicaid members in their homes and/or other settings, including community centers, hospitals, or providers' offices.

Requirements

  • Current, unrestricted RN license in the state of Texas.
  • 2+ years of experience working within the community health setting or in a healthcare role.
  • Intermediate level of computer proficiency, including Microsoft Outlook and Teams.
  • Reliable transportation and ability to travel in the assigned region to visit Medicaid members.
  • Access to a designated quiet workspace in your home with the ability to secure Protected Health Information (PHI).
  • Reside in a location that can receive a UnitedHealth Group approved high-speed internet connection.

Nice-to-haves

  • 1+ years of experience with long-term care services and support, Medicaid or Medicare.
  • Ability to create, edit, save and send documents, spreadsheets, and emails.
  • Knowledge of principles of integrated settings, including federal and state requirements.
