About the position

The Field RN, Care Manager position at Adventist Health is a part-time role focused on providing excellent patient care through collaboration and management of care plans. The RN will act as a liaison among patients, families, healthcare teams, and benefactors to optimize health outcomes, particularly for complex cases. This role requires a commitment to patient safety and adherence to organizational vaccination policies.

Responsibilities

  • Provide excellent patient care by assisting in the collaboration, development, implementation, revision, and reporting of the case management program.
  • Act as a liaison between the patient, family, nurse, physicians, multidisciplinary team, and patient's healthcare benefactor to optimize outcomes.
  • Lead the coordination of patient care with other disciplines within the care team, monitoring the appropriateness and timeliness of care.
  • Ensure the interdisciplinary care plan is consistent with the patient's clinical course, continuing care needs, and covered services.
  • Discuss with physicians the appropriateness of resource utilization, consultations, treatment plans, estimated length of stay, and discharge plans.
  • Focus on complex patients, frequent emergency department utilizers, chronic pain and substance abuse patients, and homeless patients.
  • Collaborate with acute care case managers to ensure appropriateness of ongoing care.
  • Coordinate the transfer of patients to tertiary centers, including the transfer of patient information required for continuity of ongoing treatment and services.
  • Provide oversight and collect data required for regulatory/accreditation compliance.
  • Manage frequent emergency department visitors by conducting a focus study review of previous and current admissions.

Requirements

  • Bachelor's Degree in Nursing (BSN) preferred.
  • One year of professional nursing experience required.
  • Experience in a care management role preferred.
  • Registered Nurse (RN) licensure in the state of practice required.
  • Case management certification preferred.
  • Basic Life Support (BLS OR HS-BLS OR RQI BLS) certification required.

  • Experience in care plans.
