About the position

The FIT Case Manager at LifeWorks NW's Project Network program plays a crucial role in supporting women in recovery from substance abuse, particularly within the African American community. This position serves as the primary liaison between various treatment teams and community partners, providing essential case management services to facilitate clients' successful engagement in treatment and long-term recovery.

Responsibilities

• Serve as the primary liaison between the substance abuse treatment team and the FIT for Recovery collaborative team members.
• Screen clients and determine their case management needs.
• Develop a Service Plan to facilitate each client's successful completion of their Individual Services and Supports Plan (ISSP).
• Facilitate engagement in services and maintain communication between treatment services and Outreach Workers.
• Maintain clear communication with FIT for Recovery partners, including Child Welfare, Juvenile Court, and Treatment staff.
• Provide monthly case management services to each client, including resource access and skills training.
• Complete documentation in the electronic health record system.
• Attend informational and clinical meetings as required.
• Travel between sites or in the community, including providing client transportation.

Requirements

• Substantial experience with addiction issues and understanding of the subculture.
• Extensive knowledge of navigating the social service network.
• Ability to accompany clients throughout their environment.
• Knowledge of the legal/corrections system.
• Ability to maintain confidentiality and appropriate boundaries.
• Effective communication skills in a culturally diverse environment.
• Good computer skills, including knowledge of Outlook and Word.
• Access to a car, valid driver's license, and proof of current automobile insurance.
