About the position

The Forensic Care Coordinator at Lutheran Social Services (LSS) plays a crucial role in supporting individuals found not guilty of crimes due to mental disease or defect. This position involves assessing client needs, developing and implementing service plans, and coordinating care for clients in various settings, including mental health institutions and community environments. The role is full-time, remote, and focuses on providing compassionate support to improve clients' quality of life.

Responsibilities

  • Assess client's needs utilizing required assessment tools.
  • Re-assess client needs at least annually.
  • Develop and implement comprehensive and individualized service plans in collaboration with clients and their support systems.
  • Coordinate and authorize services according to the service plan when required.
  • Provide services to assigned clients in alignment with the service plan.
  • Organize and plan work effectively, applying knowledge of community resources.
  • Work collaboratively with clients, service teams, and clinical supervisors to determine interventions.
  • Complete documentation within required timelines and maintain client records.
  • Testify at legal proceedings as required by law and/or role.
  • Coordinate and facilitate person and/or family-centered team meetings.
  • Engage in the clinical supervision process when required.
  • Attend and participate in staff development programs and professional seminars.

Requirements

  • Bachelor's degree in a relevant area of human services such as Psychology, Social Work, or Criminal Justice.
  • Valid driver's license and reliable transportation to perform essential duties.
  • Ability to read, write, analyze, and interpret documentation accurately.
  • Working knowledge of computers and electronic health records for documentation.

Nice-to-haves

  • Experience in community mental health or substance use counseling.
  • Familiarity with casework principles and methods.
  • Knowledge of social problems and their effects on individuals.
