About the position

The Certified Nurse Practitioner (CNP) at Valley Healthcare System is responsible for providing comprehensive primary care and managing the health needs of patients throughout their lifetime. This role emphasizes patient-centered, team-based care and requires collaboration with physicians to ensure effective treatment and health promotion. The CNP will perform assessments, implement treatment plans, and educate patients and their families about health management.

Responsibilities

  • Obtain complete health and psychosocial assessments including health history and physical examinations.
  • Implement treatment regimens for acute and chronically ill patients according to established protocols.
  • Institute known procedures for disease prevention.
  • Recall patients and provide information about diagnosis or therapy plans following physician-nurse appraisal.
  • Order, perform, and interpret routine laboratory tests.
  • Initiate requests for and review X-ray reports for normalcy.
  • Accept and initiate referrals for teaching and follow-up care to other healthcare team members and community resources.
  • Provide assessment, education, and management of individuals/families in family planning.
  • Develop and implement plans for health promotion and maintenance, including disease prevention and health education.
  • Co-manage care for patients with selected health complications in collaboration with the physician.
  • Participate in promoting VHcS programs through off-site health screenings.
  • Develop and periodically review protocols guiding collaborative practice.
  • Maintain and enhance knowledge relative to professional practice and continuing education requirements.

Requirements

  • Master of Science Degree in Nursing
  • Successful completion of an advanced nurse practitioner education program accepted by relevant nursing associations
  • Current license to practice as a registered nurse in the State of Georgia
  • Minimum of two years of related clinical nursing experience
  • Professional knowledge of nursing theory and practice
  • Extensive knowledge of physical assessment, differential diagnosis, pathophysiology, pharmacology, and management of patient problems
  • High level of analytic ability and attention to detail
  • Substantial interpersonal skills for patient instruction and collaboration with health team members
  • Ability to read, comprehend, and transmit detailed instructions
  • Ability to organize, prioritize, and work independently
  • High degree of integrity to maintain patient confidentiality
  • Physical ability to work and stand for extended periods.

Nice-to-haves

  • Experience in family practice or primary care settings
  • Familiarity with electronic health records (EHR) systems
  • Certification in a specialty area of nursing
