About the position

The Foster Care Case Manager at Sevita plays a crucial role in providing services to children, adolescents, and teens facing social, emotional, and behavioral challenges. This position involves coordinating the development and implementation of individualized therapeutic services, ensuring compliance with regulatory standards, and building relationships with families and external case managers. Responsibilities

  • Coordinate the development, review, and implementation of services and case management plans.
  • Oversee all services provided to individuals and their families, ensuring quality of care and compliance with contractual terms.
  • Monitor regulatory program compliance and quality of care delivered.
  • Assist in developing quality services and engaging activities that meet the needs of individuals served.
  • Build and maintain relationships with families and external case managers.

Requirements

  • Bachelor's degree in human services or a related field.
  • One year of work-related experience with individuals with intellectual and developmental disabilities or youth placed at-risk is preferred.
  • Current driver's license, car registration, and auto insurance.
  • Licensure and training as required by state, such as CPR, First Aid, and Behavioral Intervention Techniques.
  • QMRP/QIDDP certification as required by state.

Nice-to-haves

  • Experience working with individuals with brain injuries or other complex needs.
  • Strong communication and interpersonal skills.
