About the position

The Foster Care Case Manager in Hillsboro, MO is responsible for providing case management services to children entering or currently in out-of-home care. The role focuses on ensuring the safety and permanency of children by coordinating necessary services and resources for them and their families. The position requires strong decision-making skills, effective communication, and the ability to work collaboratively with various stakeholders, including families, state agencies, and community resources.

Responsibilities

  • Provide case management services for children in out-of-home care.
  • Make timely decisions regarding client services related to medical, education, social, and emotional supports.
  • Ensure emergency availability for clients 24/7.
  • Formulate permanency goals and plans for assigned clients.
  • Prepare records and documentation of all client interactions and services provided.
  • Facilitate meetings with biological parents, children, and caregivers as required.
  • Lead Family Support Team (FST) and Team Decision Making (TDM) meetings.
  • Coordinate parent/child/sibling visits as required by contract.
  • Identify and link clients with community resources.
  • Document client outcomes and progress effectively.
  • Coordinate with other service staff, the court system, and out-of-home care providers.
  • Attend court hearings and prepare necessary documents.
  • Transport clients to appointments and activities.

Requirements

  • Bachelor's degree in social work, counseling, psychology, sociology, or related field.
  • Minimum of one year of experience in protective social services or targeted case management.
  • Ability to read, analyze, and interpret professional documents and regulations.
  • Strong writing skills for reports and correspondence.
  • Ability to present information effectively to various groups.
  • Problem-solving skills and ability to make independent decisions.
