About the position

The Foster Care Case Manager at StepStone Family & Youth Services is responsible for delivering case management services, individual and family counseling, and training and supervision of foster families. This role involves developing treatment plans for children and their families, maintaining contact with foster families, and monitoring the progress of children in care. The position requires strong communication skills and the ability to work collaboratively with various stakeholders, including children, parents, and agencies.

Responsibilities

  • Deliver case management services to children and families.
  • Develop individual treatment plans for troubled children and their families.
  • Maintain weekly contact with foster families, children, and biological families.
  • Monitor the progress of children and families throughout the placement.
  • Assist with training, home studies, and evaluation of foster parents.
  • Use standardized tools to measure social and emotional progress and outcomes.
  • Prepare written treatment plans and progress reports.
  • Review referrals and match children with appropriate foster families.
  • Place children in state licensed or certified foster homes.
  • Obtain all required documentation for children in care.
  • Provide weekly documented contact and bi-weekly visits to children and foster homes.
  • Visit each assigned child at least once a month away from the foster home.
  • Prepare monthly progress reports for assigned children.
  • Provide Independent Living training and progress reports for assigned clients.
  • Coordinate visitation of children with biological parents, siblings, and relatives.
  • Provide supervised visits with parents, siblings, and relatives when required by court.
  • Act as a liaison with placing agencies, schools, and community agencies.
  • Attend court hearings and review hearings.
  • Attend staff meetings and treatment planning meetings as required.
  • Complete interviews, training, and home studies on prospective foster parents as needed.
  • Attend conferences and seminars for annual training requirements.
  • Conduct or participate in foster parent support group meetings.

Requirements

  • Bachelor's degree in social work, sociology, psychology, counseling, or related human services field.
  • Two years of experience working with children and families.
  • Strong verbal and writing skills.
  • Ability to work with children, parents, foster parents, client agencies, and the courts.

Nice-to-haves

  • Experience with a placing agency working with troubled children and families.
