Requirements

• Valid Kentucky motor vehicle operator's license

Responsibilities

• Perform clerical support duties.
• Input client information in computer and complete agency forms for intake packet, including application, consents, authorizations, etc.
• Complete agency forms needed to provide services.
• Setup new medical record charts for clients as needed.
• Collect any fees to be paid at time of visit.
• Maintain a medical records filing system and make records available to authorized staff members as needed.
• Answer telephones and schedule appointments.
• Answer inquiries for health information in client's record in accordance with agency policy.
• Prepare and complete necessary or requested correspondence.
• File agency forms and other correspondence in client's charts in a timely and accurate manner.
• Collect and distribute mail daily.
• Attend staff meetings and trainings as directed by Supervisor.
• Maintain confidentiality of all private client and agency information.
• Perform other duties as assigned by supervisor.
