About the position

The Float Registered Nurse (RN) at Minnesota Hospice is responsible for providing personal care related to the daily living activities of hospice patients. This role involves participating in the care team, coordinating medication and equipment needs, and ensuring effective communication with patients, families, and healthcare providers. The RN will also document patient interactions and implement care plans to support comfort and effective grieving for patients and their families.

Responsibilities

  • Participates as a member of the Minnesota Hospice care team and attends all IDT, educational in-services and staff meetings.
  • Maintain communication with providers and other staff members to coordinate optimal care for the patient, family and facilities.
  • Provide education regarding medication instructions to the patient, family and facilities.
  • Coordinate and communicate medication needs, orders and delivery with Nurse Practitioner and or RN Case Manager.
  • Coordinate equipment and supply needs with Assistant Director of Nursing.
  • Coordinate the delivery of equipment and supplies with patients, families and facilities.
  • Document all PRN visits, phone calls and client interaction in electronic health record within 72 hours.
  • Implement care to achieve outcome related to comfort and safe dying, self-determined life closure and effective grieving for the patient, family and facility.
  • Maintain communication with facility and families through the hospice binder calendar and visit record note entries at each visit.
  • Communicate changes in patient's condition to the RNCM and Director of Nursing.
  • Administer medication and treatments as ordered by providers.
  • Ability to recognize client's psychosocial needs and report to RNCM, Social Work and/or Chaplain as appropriate.
  • Perform clinical tasks such as enemas, wound care, catheters, ear lavage, etc. as needed.
  • Provide emotional and psychosocial support when needed.

Requirements

  • Graduate of Accredited Nursing Program
  • Possess a valid Minnesota Registered Nurse (RN) License
  • Current CPR/BLS Certification
  • Minimum of 1 year nursing experience in healthcare setting
  • Knowledge of appropriate HIPAA and confidentiality
  • Ability to accurately use the PAINAD scale
  • Maintain accurate documentation
  • Able to work as both a part of a team and independently
  • Possesses great organizational skills
  • Excellent time management skills.
  • Valid driver's license and auto insurance
  • Knowledge of Microsoft products i.e. Excel, Word, etc.

Nice-to-haves

  • One year hospice or long term care experience preferred
  • Willingness to obtain the certification in Hospice and Palliative Care within 2 years of employment.
