About the position

Capstone Hospice is seeking a full-time Registered Nurse (RN) Case Manager to join their dedicated hospice team in the Sugar Hill/Gainesville area. The RN will manage a designated caseload of patients in their homes and assisted living facilities, focusing on symptom management, family education, and regulatory documentation. This role requires independent work and participation in bi-weekly interdisciplinary meetings, with a commitment to providing high-quality hospice care.

Responsibilities

  • Manage a designated caseload of 10-14 hospice patients in home and community settings.
  • Observe and report any unexpected changes in patients' physical or emotional conditions to the physician and team members.
  • Teach, supervise, and counsel patients and their families on care provision.
  • Organize work schedule to attend all required meetings and manage time effectively.
  • Develop and re-evaluate patient/family care plans in conjunction with the interdisciplinary group (IDG).
  • Document patient visits in the electronic medical record system (Wellsky Consolo).
  • Perform specific nursing procedures as needed, following doctor's orders.
  • Coordinate the implementation of care plans for patients in skilled nursing facilities (SNF) or assisted living facilities (ALF).
  • Follow hospice policies and procedures, ensuring confidentiality and compliance with HIPAA regulations.
  • Comply with infection control policies and protocols.

Requirements

  • Graduate from an accredited school of nursing.
  • Currently licensed as a registered nurse in the state of Georgia.
  • Minimum of 2 years of experience as a registered nurse.
  • Active patient contact within the past three years is preferred.
  • Must provide evidence of annual TB test and other state-required tests or examinations.
  • Must have a valid driver's license, auto liability insurance, and reliable transportation.
