About the position

Visiting Nurse Home Care & Hospice of Carroll County is seeking a compassionate and driven RN Case Manager to provide the highest-quality care for patients in their homes in a one-on-one setting. This full-time position (32 hours per week) offers flexibility for those who may want to work more than 32 hours but no more than 40 hours per week. The role is crucial in ensuring that patients receive comprehensive care tailored to their individual needs. As an RN Case Manager, you will be responsible for managing patient admissions, care management, and follow-up visits, utilizing the nursing process to assess, diagnose, plan, implement, and evaluate care. In this position, you will develop and update care plans in collaboration with an interdisciplinary care team, ensuring that all aspects of a patient's health are addressed. Your nursing care may include physical assessments, medication management, wound care, IV therapy, tube feeding, and education for both patients and their families. You will coordinate care with various healthcare professionals, including physicians, nurse practitioners, and therapists, to ensure a holistic approach to patient care. Regular communication with patients' physicians regarding care needs and progress is essential, as is compliance with HIPAA guidelines and state and federal regulations. Documentation of clinical notes must be clear and concise to maintain accurate patient records. Responsibilities

  • Provide admission, care management, and follow-up visits for patients using the nursing process (assess, diagnose, plan, implement, evaluate).
  • Develop the plan of care for new patients and update the plan with the interdisciplinary care team.
  • Provide nursing care, including physical assessment, medication management and administration, wound care, IV therapy, tube feeding, and durable medical equipment provision and use.
  • Coordinate care with physicians, physician assistants, nurse practitioners, home health aides, physical therapists, occupational therapists, and other clinicians.
  • Regularly communicate patient care needs and progress to each patient's physician.
  • Educate and assist patients and their families in proper home health care procedures and strategies to keep the patient safe at home.
  • Remain in compliance with HIPAA guidelines as well as all state and federal regulations.
  • Document clinical notes in a clear and concise manner.

Requirements

  • Must be a registered nurse in New Hampshire or Maine.
  • Strong organizational and communication skills.
  • Reliable transportation.
  • Valid driver's license and auto insurance (minimum limits of $100,000/$300,000).
  • Clean background checks will be required.
  • Availability to rotate on weekends and holidays on a limited basis each year.

Nice-to-haves

  • Prior home health experience preferred but training is available.
