About the position
The Licensed Practical Nurse Assessment Coordinator (LPNAC) at Hamilton Arms Center is responsible for leading the MDS team in conducting resident assessments and ensuring compliance with Medicare, Medicaid, and state regulations. This full-time, on-site role requires strong assessment skills and familiarity with billing processes, aiming to enhance the quality of care provided to residents.
Responsibilities
- Conduct and coordinate the completion of the resident assessment in accordance with state requirements and facility policies.
- Conduct and coordinate the completion of the RAI process in compliance with Medicare, Medicaid, OBRA, and other payer programs.
- Develop, implement, and maintain an ongoing quality assurance program for resident assessments.
- Participate in facility surveys conducted by authorized government agencies.
- Monitor the facility's QI and QM reports to ensure appropriate corrective actions are taken when potential issues arise.
- Provide initial and ongoing education to the interdisciplinary team involved in the RAI process.
Requirements
- Licensed Practical Nurse (LPN) with effective leadership and assessment skills in a long-term care setting.
- Previous experience as a LPNAC and in conducting assessments.
- Knowledge of state and federal regulations related to long-term care.
- Proficient in PA CMI.
- Experience with Medicare billing and knowledge of PPD (PDPM).
- Experienced in insurance updates.
- Must successfully complete a post-offer, pre-employment criminal background check and physical/drug screen.
