We are currently seeking a RN Case Manager in the Dallas/Fort Worth area who shares our passion for providing dignified, compassionate end-of-life care. This person will be covering the Fort Worth area. Shift Schedule: Day Shift, Monday - Friday.

Responsibilities

  • Develop a detailed Plan of Care for assigned patients ensuring all services needed are identified and met while working closely with the Interdisciplinary Group, patient, family and attending physician
  • Responsible for the coordination of care for the patient and family, providing skilled Hospice and Palliative care for the remainder of patient's life
  • Provide comprehensive nursing evaluations and assessments
  • Provide field supervision to LVN's and Home Health Aides assigned to your patients
  • Filed case management
  • Other duties as assigned

Requirements

  • Must be a Registered Nurse with a valid state license
  • One year of experience working with your RN license required
  • Hospice experience preferred, but not required
  • This role requires a valid driver's license and a good driving record as a condition of employment
  • Reliable transportation and willingness to drive to patient's location
  • Must be able to lift 50 pounds
  • Must clear a criminal background check, physical and drug screening
  • You may be required to work outside of your regular hours depending on business needs
