About the position

The Full Time Licensed Practical Nurse (LPN) at UPMC Horizon Resource Pool plays a vital role in providing nursing care under the supervision of a Registered Nurse or other licensed provider. This position involves participating in the planning, implementation, and evaluation of patient care, ensuring adherence to UPMC LPN Practice Guidelines and state regulations. The LPN is expected to communicate effectively with the healthcare team, provide patient education, and contribute to a compassionate patient experience while maintaining a focus on safety and quality of care.

Responsibilities

  • Participate in the planning, implementation, and evaluation of nursing care using focused assessment.
  • Communicate with licensed professional nurses and healthcare team members regarding patient care needs.
  • Obtain instruction and supervision for new or unfamiliar nursing practices or procedures.
  • Follow established policies and procedures of UPMC and the State Practice Act.
  • Formulate daily goals and a plan of care for patients, involving them as partners in their care.
  • Document patient conditions comprehensively to promote communication between caregivers.
  • Create a caring and compassionate patient-focused experience by building relationships with patients and families.
  • Provide detailed teaching to patients and families to guide them through their care.
  • Support the orientation and development of new LPNs and patient care support staff.
  • Identify opportunities for clinical quality and workplace improvement.

Requirements

  • Successful completion and graduation from a Licensed Practical Nursing Program approved by the State Board of Nursing.
  • 0 - 2 years of experience for LPN; 2 years of experience for Senior LPN.
  • Successful completion of an IV therapy curriculum required within 4 months of hire/transfer.
  • Ability to demonstrate knowledge and skills necessary to provide care for patients of various ages.
  • Ability to establish and maintain positive relationships with healthcare team members and patients.

Nice-to-haves

  • Experience in a med/surg/tele or Emergency Department setting.
  • Certification in a UPMC approved program for patient care improvement.
