About the position

The Long-Term Care Functional Screen Specialist plays a crucial role in supporting the Inclusa/Humana team by assessing the functional status and ongoing eligibility of members in the Wisconsin Family Care program. This position involves conducting face-to-face interviews, utilizing a state-designed screening tool, and ensuring accurate documentation and referrals as necessary. The specialist will work closely with member support coordination staff and participate in quality assurance activities to maintain high standards in functional assessments.

Responsibilities

  • Schedule appointments with members for annual re-determination of functional eligibility.
  • Use the DHS screening tool to assess individual functional abilities and health status.
  • Refer members to appropriate staff or resources based on identified needs.
  • Respond to urgent situations and facilitate crisis intervention as needed.
  • Maintain up-to-date online member information regarding functional abilities and demographics.
  • Conduct face-to-face interviews in members' homes to perform functional screens.
  • Enter assessment data into an internet-based program to calculate levels of care.
  • Provide notice of member rights regarding changes in level of care.
  • Submit timely and accurate documentation according to established guidelines.
  • Attend meetings and training to stay updated on agency processes and screening tools.
  • Participate in quality assurance activities to ensure competence among staff.
  • Maintain certification for functional eligibility screens through ongoing training.

Requirements

  • Four-year bachelor's degree in a Health or Human Services related field with one year of experience with Family Care target populations, or a Wisconsin licensed Registered Nurse with one year of experience with Family Care target populations.
  • Certification as a Long Term Care Functional Screener (LTCFS) after completing required training.
